75% chance that if he hasnt been hit in the nose lately, its just his esophagus bleeding. Its pretty delicate, and severe coughing can tear it up pretty quickly, as can vomiting, if you've been doin that lately too.
EDIT: After re-reading that, it sounds severe. When I say bleeding, I just mean it's been irritated, not like you got your throat slit and now you're gonna die :P
