I think one major problem is no unified Nation-world wide commission. We have States that actually 'borrow' them because they lack their own. Lots of craziness in Argentina, I remember seeing a fight where the ref literally held up an obviously badly hurt concussed fighter because he was a the heavy favorite and champion. A crooked official doing favors can get fighters severely damaged or worse.
