Anyways on to the main argument I see it the same as calling someone a 'dick' or 'dickhead'.

If he were to call you a dick head and then later to call you a dick, I think that's perfectly grammatically acceptable as both mean the same thing, and hence can be interchanged.

If on the other hand he called you a 'c*** face' and then a 'dick head' then it would be illogical as you couldn't have both a vagina and a dick on your face. I guess he could also have called you 'S*** head' and then the implication would be that you had both a cock, a vagina and an anus all on your face'
