Yes there would have to be rule changes to take into account either fighter getting cut. But the actual chances of becoming HIV+ in these controlled circumstances would be minimal.
The real reason it probably could not happen is the extra insurance costs for each fighter.

The truth is Tua's reluctance to step into the inside of Lewis' jab, was the main reason the fight was considered boring.
