Golota. Headbutts, COMBINATION groin-punching, Dracula-like biting of an opponents neck. The guy was talented but didn't have the mentality to be in the ring.

(Dis)honorable mention goes to Tyson. Ear biting. Headbutting. Trying to break opponent's arms in clinches. Hitting Lou Savarese after the ref jumped in (and hitting the ref with a forearm/bicep). Biting Lewis' leg at a weigh-in.

Also that James "Hammer" Butler guy. The fella who murdered Max Kellerman's brother. I haven't seen many of his fights, but just for that one sucker punch on Richard Grant after their fight he deserves to be mentioned here IMO.

Not sure about all time, but Robert Guererro is DEFINITELY a dirty fighter. I like that though.

Also, may he Rest in Peace, but Agapito Sanchez was the definition of dirty. He used headbutts, elbows, lowblows everything. His fight with Pacquiao is one of the worst I've ever seen really. That could have been called easily in the second round. And if not the second, definitely the third.
