What is a sparring form? Do you mean headgear or a mouth piece?

He may mean a medical and a boxing card. People aren't supposed to be sparring around here w/out a medical. Although some do.

Either that or he's just messing around. No such thing as a sparring form. There is the Roy Rones Junior sparring form (technique) but it will generally just get you laughed at and eventually catching a lot of hard hooks.

The boxing medical form should be available from the club.

I don't mean to worry you, but if the club did not distribute sparring forms then they probably haven't been covered by any health and safety laws up until this point.
It went into legislation before the initiation of last years European season that all boxers should be medically checked and ABA (or in my case IABA) carded before participating in sparring matches.
    Check that out.

And if the club doesn't have any perhaps attend yourself to see how the club is run.
