Chiang Mai is great, much cooler and wetter than the rest of Thailand. If your girlfriend is from there, she will know the city really well. You could get a genuine Sak Yant tattoo done and blessed by a Buddhist monk while you are there ;D;D
Bangkok is mad, vibrant and fun. As you're s boxing fan you should try and get tickets to the Lumpini Stadium to watch the Thai boxing. If you go with your girlfriend you could get into the Thai area (they always sequester foreigners in a separate area, and I've always wanted to sit with the Thais as they are raucous and knowledgeable). Lumpini is the Madison Square Garden of Thai boxing and you will see some talented and unbelievably brutal fighters!
I have only spent a little time in Pattaya, but I enjoyed Phi Phi and Ko Lanta more as laid back beach places. Khao Lak was cool too.
Youre right about the bikes, but seriously mate the roads are a death trap on a bike or scooter. Really. I have lost track of the number of foreigners I have seen with injuries and big bills.
