Go back and watch old fights, from the 1920s. They fought with 4-6oz gloves- and you bet your ass you slipped, ducked and parried more punches than you blocked. No 'ear-muffs' then. Gunboat Smith said that, in his era, they wrapped their hands with packing tape and wore 2 oz gloves; they sparred with 8s. watch those old fights and notice the amount of backhands; it was a very effective weapon.
The number of kos was probably lower than it is today because guys hurt their hands more often- they weren't as protected so they weren't swinging for the fences all the time. Also, when you swing and miss (like when your opponent ducks or slips) it takes more out of you.
Off the top of my head...
Baer- the guy that does not look like Jack Dempsey but was trained by Dempsey for a spell- uses just about all of the punches you mentioned.
