Gunboat Smith said that, in his era, they wrapped their hands with packing tape and wore 2 oz gloves; they sparred with 8s. watch those old fights and notice the amount of backhands; it was a very effective weapon.
The number of kos was probably lower than it is today because guys hurt their hands more often- they weren't as protected so they weren't swinging for the fences all the time. Also, when you swing and miss (like when your opponent ducks or slips) it takes more out of you.
